Prior to Your Visit
We require that all new patients complete a set of new patient forms prior to your visit.

Please print the forms from our Web site and fill them out before your visit. Visit the New Patient Forms area of our Web site to begin. Bring the completed forms to your first appointment or fax the forms to: NEW PATIENT 404-355-6980.

It is important to be on time for your appointment. If you are late, your provider may not be able to see you for your appointment.

Please bring the following information to your first visit (if available):

  • Patient information (address, phone number, age, date of birth)
  • Copy of insurance card
  • Treatment notes (history and physical, and last three progress notes from your physician)
  • Diagnostic reports (including X-rays, CTs, MRIs, EMGs. Bring actual films to your first visit)
  • Any other medical information you feel may be of assistance
